Animal Hospital Tier List

Animal Coins are slow to earn, so spend them on classes that actually carry a shift. Below: the best classes ranked, plus a danger tier list for the threats you'll face each night.

Rankings reflect the June 2026 Class Update and shift with every balance patch. Classes have three upgrade levels each; perks scale as you level them. Verify perks in the live game after any update.

Class tier list

TierClassUnlockWhy it ranks here
SSurgeonAnimal Coins (F2P)Boosts Sanity and movement speed — the two stats that matter on every shift. Best free pick.
SHead NurseRobuxIncreased inventory capacity; carry more coffee, meds and tools into a run. Premium powerhouse.
ASecret AgentRobuxStarts with a gun — cleans up an admitted Skinwalker fast. Insurance for late shifts.
BSecretaryAnimal CoinsRecovers Sanity each time you check in a patient — a cheap reception pick that pays off on long shifts spent at the desk.
BDefault / InternStarterNo perk, but the game is winnable on skill alone. Fine until you can afford an S-tier.

More classes exist in-game and the roster is expanding via the Class Update line. We list only the roles we can confirm from current sources; check the status hub for the latest additions.

Spending priority: if you're free-to-play, save Animal Coins for Surgeon first. The speed + Sanity combo prevents more deaths than any combat perk, because the best counter to a Skinwalker is never admitting it.

Anomaly & threat danger tier list

How dangerous each threat is once it's loose in the clinic, and how easily it ends a run:

TierThreatDanger
SSkinwalkerAn admitted anomaly converts and hunts staff and patients; melee drains Sanity fast. Can wipe a shift. Guns/Tasers are the only reliable counter.
AStalkerSpawns around corners; never look directly at it or it drains Sanity. Weapons don't work — break line of sight and keep moving.
ATentacles / Bed MonsterRoom hazards that turn a treatment station into a kill zone — back away, don't treat it like a normal bed.
BFire / SlimeEnvironmental room hazards; easy to read, punish you only if you stand in them.
BInfectorSpread/contact threat — avoid direct contact, handle as an infection hazard.
CCamera eventsCCTV scares that advance the run if ignored; read the feed and react.

Remember: Shift cash buys Supplies Shop tools during a run; Animal Coins unlock classes in the lobby between nights. Don't confuse the two economies. New to the systems? Read How to Play.

Frequently asked questions

What is the best class in Animal Hospital?

Surgeon is the best free-to-play class — it raises Sanity and movement speed, the two stats that matter on every shift. Head Nurse is the top premium pick for its extra inventory capacity.

Should I spend Animal Coins or Robux on classes?

If you're free-to-play, save Animal Coins for Surgeon first. Robux classes like Head Nurse and Secret Agent are optional power picks — the game is winnable without them.

What is the most dangerous threat in Animal Hospital?

The Skinwalker. An admitted anomaly converts into one and hunts staff and patients; the only reliable counter is never admitting it, with a Taser or Gun as backup.

What does the Secretary class do in Animal Hospital?

The Secretary is an Animal Coins class that restores a little Sanity every time you check in a patient. It's a cheap, low-risk pick for whoever holds the reception desk on long shifts.
