- Check in patients at the reception desk — each is a normal animal or an anomaly in disguise.
- Run the five-point check: your eyes, the check-in photo, the CCTV feed, the paperwork and the audio.
- Admit or reject — hit the red Shutter for anything suspicious. Rejecting a real patient is safe; admitting an anomaly is not.
- Treat the real patients across the rooms — Basic Medical/DNA, X-Ray, Heart Monitor and Surgery.
- Manage Sanity with coffee and food, survive timed events, and reach the end of the shift for cash.
