How to Play Animal Hospital
Animal Hospital is a night-shift clinic survival loop. The game punishes rushed admissions more than slow play — one wrong admit can stack with treatment timers, Sanity drain and enemy events. Here's everything you need for your first shifts.
The core loop
Every night repeats the same rhythm: check in patients at the reception desk, decide admit or reject, treat the genuine patients across the rooms, manage your Sanity, and survive timed events until the shift ends. The more anomalies you reject and patients you cure, the higher your end-of-shift score and the more cash you earn.
- Inspect each visitor with the five-point check (see How to Spot Anomalies).
- Reject with the red Shutter button if anything is off; admit genuine patients.
- Route & treat the real patients — Basic Medical/DNA, X-Ray, Heart Monitor and Surgery rooms.
- Recover Sanity with coffee and food between arrivals.
- Survive events and emergencies, then bank your shift score.

Cash vs Animal Coins
Two separate economies — don't confuse them:
- Shift cash buys Supplies Shop tools and upgrades during a shift (coffee, medkits, weapons).
- Animal Coins unlock classes in the lobby Class Shop between nights. Save them for an S-tier class — see the tier list.
What "utility" means
Players use utility as a catch-all for support and quality-of-life upgrades — the ones that make a shift smoother rather than directly boosting Sanity or fighting anomalies. Think extra inventory space, faster patient check-in, quicker DNA analysis and faster patient recovery. The standout utility buy is the Check-in Window Upgrade, which adds a second window so two patients can be processed at once.
How many shifts & the ending
Animal Hospital has 5 main story shifts, and the difficulty climbs sharply each night — Barney and Ratthew become factors on the later shifts, and the true-ending route plays out on Shift 5. The game doesn't stop there: past the story it continues into endless, escalating nights with no hard cap, which is where score-chasing teams push to Shift 10 and beyond.
Sanity is your real health bar
Sanity drains when an anomaly attacks you or a patient, when you stare at certain camera feeds, and when you look at cursed photos. It hits zero, your run ends. Restore it with coffee and food from the shop, and keep a coffee ready before risky events like the Death Ritual.

Once you're admitting confidently, move on to the Treatment Guide and keep the Anomaly Identifier open while you play.
