Animal Hospital Characters
Animal Hospital fills its night shift with animal faces, and working out who actually wants to help you is half the job. Some keep the hospital running, one will kill you if you say the wrong thing, and a few just sit in the lobby. Here's every named character and what they do.

Dr. Harlow — the doctor who runs your shift
Dr. Harlow appears in every shift and is the closest thing you have to a guide. He shows up at the end of each shift to grade how you did, and his real value is what he unlocks over time: after Shift 1 he opens the Shutter and cameras, in Shift 2 the shop and the emergency rooms (Rooms 6–8), and in Shift 3 the Taser. There's also a quiet hint he may be more than he seems — an anomaly version of him can be spotted in one of the lobby windows.
Barney — the serial killer you have to handle carefully
Barney debuts in Shift 3 and is the one character you should genuinely fear. He looks like a friendly white rabbit with a brown hat, a mustache and plaid pants, but his anomaly form swaps that for a wide, sharp-toothed mouth and big hollow eyes. He's an anomaly and a serial killer who carries a kitchen knife — and he's already used it on Officer Duckman.
With the police closing in, Barney asks you to hide him in the office. He's polite at first — calling you "friend" — but if he thinks you might report him, he warns that "friends do not turn each other to the police" and threatens to kill you. You have three options:
- Hide him — he thanks you and hands over his suitcase, which holds a second coffee machine.
- Turn him in — Officer Duckman arrests him, but only after Barney knocks the officer out and flees.
- Reject him outright — close the Shutter or let his timer run out.
Ratthew, Ron & Officer Duckman
Ratthew is kinder than he looks. He's cautious and hides who he really is when you first meet him, mostly because past experiences left him distrustful — stick with him and the friendlier side comes through. He is not a patient anomaly you reject. Ron from Accounting is your blunt source of information: he understands the anomalies and monsters and tells you about them without softening the threat. Officer Duckman is the police officer tracking Barney, and his fate is tied directly to your decision to hide or report him.
Frequently asked questions
Who is Barney in Animal Hospital?
Barney is a character and anomaly who debuts on Shift 3 — a white rabbit in a hat who demands coffee and turns hostile. He's a serial killer with a knife who asks you to hide him from the police; close the Shutter or let his timer run out if you'd rather not deal with him.
Is Ratthew an anomaly?
No. Ratthew is a named, kind-hearted character who is wary of you at first and hides his identity because past experiences left him distrustful — not a patient anomaly you reject.
Should you hide Barney or turn him in?
Both are valid. Turn Barney in and Officer Duckman arrests him after Barney knocks him out; hide him and he thanks you and hands over a suitcase containing a second coffee machine.
