  • The Stalker — a tall shadowy figure that drains Sanity if you look at it. People read its silhouette as "a ghost" in dark hallways.
  • An anomaly that got inside — admit the wrong patient at the desk and it converts into a Skinwalker that hunts the halls. Some players call that loose hunter "the ghost" too.

How to get rid of each, step by step

Hiders (the real "ghost")

Hiders are why the ghost feels invisible. Track them by sound — the raspy breathing gets louder as you near one — and by the glowing eyes that give away an otherwise blank patch of wall. Once you've found it, the reliable kill is a taser or gun. With no weapon, you can hold E from behind it, but melee bleeds your Sanity, so it's a last resort.

Sources conflict on the fire extinguisher. Several guides say spraying the fire extinguisher reveals or drives off invisible monsters like Hiders; another says the extinguisher is only for room fires and burning patients. Until we can confirm it in the live game, treat the extinguisher as a maybe for revealing a Hider — but rely on the taser or gun to actually remove one.

The Stalker

The Stalker can't be shot or tased — fighting it just feeds it. The counter is avoidance: don't look directly at it, break line of sight, and keep moving. Looking drains Sanity, and Sanity hitting zero ends your run.

A loose Skinwalker

If you wrongly admitted an anomaly, it can convert into a Skinwalker that roams the halls. Use a taser or gun to put it down, then treat the root cause — over-rejecting at the window is always safer than admitting one hostile.

Barney (the coffee monster people fear)
